Make a referral

Below are referral forms for each of our service streams. These forms allow you to refer yourself, or someone else, for a Scope Home Access service.

To complete the referral, you will need to provide details about the person the referral is for, including their personal information and contact details. If you are a service provider, you must have the consent of your client before making a referral.

Once submitted, our intake team will be in touch within 1-2 business days to confirm receipt of referral.

Aged Care Services (CHSP) Referral Form

If you, or the person you're referring, are registered with My Aged Care and eligible for funding under the Commonwealth Home Support Program (CHSP), use this form to refer for funded Aged Care services.

Disability Services (NDIS) Referral Form

If you, or the person you're referring, are a participant of the National Disability Insurance Scheme, use this form to refer for Disability services.

Social Enterprise Referral Form

If you, or the person you're referring, are not eligible for funded services, or are the recipient of a Home Care package, use this form to refer for Occupational Therapy, Home modification or Home and Yard maintenance services.
